A Star provide community based support for people with a range of learning & physical disabilities across North Wales. We understand that enjoying a life that has purpose and meaning is equally important to us all. People with learning disabilities are no exception. Our services are designed to provide care in the community, supporting people with learning disabilities to live ordinary and fulfilling lives.

 

Personalised Disability Support

 

At A Star, we support you to live the life you want, where each day can be different. Our staff recognise each person as an individual with a range of unique personal interests and aspirations. By focusing on your unique strengths and abilities, our aim is to help you achieve your personal goals. We develop a personalised support plan with you, so you can receive your care in the community where you live.

Our servcie helps people to achieve the following:

  • Keep connected – meet friends and family, go to school, college or clubs, pursue a favourite hobby
  • Keep fit and healthy – attend medical appointments, eat well or go to a fitness centre
  • Stay independent – manage money, take a shopping trip to town or pay bills
  • Maintain a daily routine – get up and go to bed
  • Keep your home clean and tidy – help with domestic chores like washing and ironing

Who this service is for

This service is avaliable for people aged 18+ who may have any of the following:

  • Learning Disabilities
  • Emotional difficulties
  • Mental health conditions
  • Wide range of medical conditions

What's on offer

We will encourage and provide individuals with opportunities to be involved in activities they are interested in by identifying their hobbies through the Person Centered Plan/Personal Plan. Based on plan assessment outcomes, we then build these activities into individual weekly Active Support Programme.  Service user have access to the Clock Tower Facilities which is an ASDAN acrredited centre where people undertake qualifications in a wide range of subjects such as Life Skills, Independence Skills, Hobbies, Interests, Getting ready for work, Personal Safety, DIY.  There are over 180 subjects available.

Physically Fitness

We understand the many benefits that regular excercise has both to a persons physical fitness but also their mental health. We encourage use of the gym, bike riding, swimming, walking, dancing, football, wii fit and recording daily exercise. We have a number of facilities locally that enable us to provide a broad range of activities. We also by encourage a healthy eating diet and provide healthy eating menus.

Mental Health

We have a well established Mental Health monitoring system that frequently assess’s individual mental well being.  If appropriate supporting people to appointments and working closely with the Learning Disability Psychiatric Nurses. By giving the residents the opportunity and encouragement and the environment where people feel comfortable talking about their feelings. Facilitate any necessary referrals to medical professionals.

Emotional Needs

As part of the Person Centered Plan we complete Emotional Needs assessments for all residents which help us to identify the individuals needs so we can then ensure that the service we deliver supports the individuals specific requirements.

Active Support Plan

Plas Ucaf has an Active Support Plan which the individual residents participate in completing on a weekly basis. This helps the person have control over their everyday life, the key decisions and any individual choices can be catered for.

Advanced Support Needs

During the initial needs assessment, If an individual requires more advanced support than we have provision for, we can arrange for this to provided. Typically, these are the following main areas of external body support available:

  • Neuropsychologists
  • Clinical psychiatrist
  • Occupational therapists
  • Physiotherapists
  • Speech and language therapists (SaLTs)
  • Qualified nurses
  • Therapy assistants

Support Staff

A Star pride ourselves on providing support staff that are employed only after a thorough recruitment and vetting process.

Staff are encouraged to commence QCF II or III, as soon as they start their employment with A Star and at least 80% of our staff have got this qualification or are working towards it. We also ensure that our staff do bespoke training if there is a requirement.We undertake person specific training, but in general, all our staff complete the following:
